Your HUSKY Health Member ID Card

When you join the HUSKY Health program, we will send you your blue and white HUSKY Health member ID card in the mail.

The HUSKY Health ID card has your name, member ID number, and the name of your HUSKY Health program (A, B, C, D, or LB). There are also helpful phone numbers on the back of the card.

HUSKY Health does not send out new HUSKY Health ID cards to members each year. You will receive your initial and only HUSKY Health ID card when you join the HUSKY Health program. You can always request a replacement ID card if your card is lost or stolen.

Need a replacement HUSKY Health ID card for a child/dependent?

If you're looking to view or print a HUSKY Health member ID card for someone in your household who is under 18 years of age, you must submit a “Child/Dependent Account Registration” form. This form can be found by logging into your secure member account.

You will need to fill in the dependent’s first and last name, date of birth, and HUSKY Health member ID number. Please allow three business days for the form to be reviewed. Once it’s approved, you will be able to view and print the HUSKY Health member ID card(s) for your underage dependent(s). Members who are 18 years of age or older must create their own account to view and print their member ID cards.

How long does it take to get a replacement ID card?

If Member Engagement Services mails you a replacement HUSKY Health member ID card, it will take 10 - 14 business days. If you want to view or print your HUSKY Health member ID card online, you can do that right now from your secure member account!

Where do I get the gray ConneCT card?

The gray ConneCT card is sent to you by the Department of Social Services (DSS). If you have not received your gray ConneCT card, need a replacement, or have any other questions about this card, please call the DSS Client Information Line and Benefits Center at 1.855.626.6632.
